Transportation:

Departure from CPS: 5pm - Bus will leave promptly at 5pm

Attire:

Shoes that SLIDE and dresses, skirts or dressy pants, and button-down shirts or blouses.
Sorry, but no sneakers, sweats or jeans – look sharp!

Payment

​The cost for each student is $200. This covers four lessons and transportation to and from the studio.

REGISTRATION FORM & PERMISSION NOTICE

In submitting this form and paying the fee I give permission for my child, named in this registration form, to participate in the Sixth Grade Social Dance and hereby release Ellen’s School of Dance and Class of 2027 organizers from any and all claims arising out of injury that may be sustained by my child while participating in this activity.

Chaperones

We're looking for four parent chaperones at each dance lesson! This is a great opportunity to see the students in action as they learn formal dances–an experience not to be missed. If you're available to assist, please fill out your availability below or email Stephanie Koch.

Chaperones must have an updated CORI on file with Carlisle Public School.
